Twin-Star recalls space heaters due to fire and burn hazards

The Duraflame electric space heaters have been recalled by Twin-Star due to the fact that the heater can overheat and cause the units to melt, catch fire and ignite nearby items. Approximately 31,000 of these units have been recalled because they pose a fire and burn hazard to consumers.

Twin-Star has received 32 reports of the heaters burning or melting, including eight of which have caught fire, one resulting in property damage. Consumers should immediately turn off and unplug the electric space heater and contact Twin-Star at (855) 884-9651 for a full refund. Rollerblade USA recalls skates due to fall hazard

The Rollerblade® Tempest Inline Skates have been recalled by Rollerblade USA at the amount of 11,800 pairs of skates. Reportedly, the mounting holes in the boot and frame can be misaligned. This can cause the boot to separate from the frame, posing a fall hazard.

Packaging Tape Inc. recalls air movers due to fire hazard

Approximately 2,800 units of the Air Movers/Blowers from Packaging Tape Inc. have been removed due to fire hazard. Reportedly, the switch of the product can fail and cause the motor to overheat, posing a fire hazard. There have been eight reported incidents, including four reports of fire, one of which caused $225,000 in property damage. BMC recalls bicycles due to crash and injury hazards

BMC has recently recalled 1,300 units of their Stromer Electric Bicycles in the United States; and 11 in Canada. The recall stemmed off of a hazard that the bicycle fork can break, posing a crash and injury hazard to the rider. Belmont Chocolate Chip Cookie Dough Recall

House of Flavors Ice Cream Company is issuing a voluntary recall for all Belmont Chocolate Chip Cookie Dough due to the possible nut allergens in the product. The product was distributed in Michigan, Ohio, Indiana, and Illinois. There have been two documented cases of allergic reaction due to consumption of the product by allergic consumers. Crown Foods recalling “Golden Natural Fruit Island”

Crown Foods is issuing a recall for “Golden Natural Fruit Island” for sulfites contained in the product that were not declared on the packaging. Sulfites can pose a serious health risk to those with allergic reactions. Undeclared yellow C coloring was also found in the product. Pfizer Inc. Effexor XR ® (venlafaxine HCl) Recall

Pfizer Inc. is voluntarily recalling Greenstone LLC-branded Venlafaxine HC1 150 mg extended-release capsules. A pharmacist had reported that a bottle of Pfizer’s Effexor XR contained one capsule of Tikosyn® (dofetilide) 0.25mg in addition to the Effexor XR capsules. Although Pfizer did not receive any other reports of poor packaging, these three lots are being voluntarily recalled as a precaution because they were packaged on the same line.

The use of Tikosyn by an Effexor XR/Venlafaxine HCl patient, where the contraindications and drug-drug interactions with Tikosyn have not been considered by the prescribing physician, could cause serious adverse health consequences that could be fatal.

There is a very low probability that other bottles of Effexor XR contain a Tikosyn capsule, Pfizer has initiated this voluntary recall as a precaution.Effexor XR is a prescription antidepressant indicated for the treatment of major depressive disorder, general anxiety disorder, social anxiety disorder, and panic disorder with or without agoraphobia. Tikosyn is a Class III cardiac antiarrhythmic drug. It is used to treat irregular heartbeats and to maintain normal rhythm.

This recall extends to the patient level and involves Pfizer lot numbers V130142 and V130140, which both expire in October 2015, and Greenstone lot number V130014, which expires in August 2015.

These products were distributed nationally to wholesalers, distributors, certain government agencies, patient assistance programs and retailers, such as pharmacies and hospitals. These direct customers are being notified by UPS next day mail, and Pfizer is arranging for the return of all recalled products.

Pharmacists should immediately quarantine, discontinue distribution of and return all recalled lots of these products, as well as notify any of their customers to whom they distributed the products. Patients with affected product should notify their physicians and/or return product to their pharmacies.
